Birth & Adoption Registration

1. Birth in a Health Facility

• 🏥 Notification of birth from the health facility.
• 🆔 Copy of parent(s) IDs.
• 📝 Duly filled-in Form 3, signed by the declarant.
• 🏦 Bank payment slip in the name of the child, where applicable.

🎫 Document issued: Birth Certificate.

2. Birth in a Community

• 🆔 Copy of parent(s) IDs.
• 📝 Duly filled-in Form 3, signed by the declarant.
• 🏦 Bank payment slip in the name of the child, where applicable.

🎫 Document issued: Birth Certificate.

3. Birth of a Citizen Occurring Outside Uganda

• ✉️ Application letter to the Executive Director.
• 📜 Statutory Declaration registered with URSB.
• 👶 Copy of birth certificate from country of event (must be in English).
• 🆔 Identification (NID or Ugandan passport) of the declarant.
• 💰 No fees payable.

🎫 Document issued: Confirmation of registration letter.

4. Adoption Registration

• ⚖️ Certified or original Adoption Order.
• 🆔 Copies of parent(s) identification (ID or passport).
• 👶 Copy of child's Birth Certificate.
• 💰 Fees: Nil.

🎫 Document issued: An Adoption Schedule.

5. Registration of a Guardianship Order

• ⚖️ Original or certified copy of the Guardianship Order issued by a court of competent jurisdiction.
• 👶 Birth Certificate of the child.
• 🆔 National Identification Card of the guardian.
• 🪪 NIN / Card of the child, where one has been issued.
• 🆔 National Identification Cards of the biological parents, where available.
• 💰 Fees: Nil.

🎫 Document issued: A confirmation of registration letter.
