Pre-registration lets you enter your information from anywhere, upload supporting documents, and receive a reference number to show when you visit. It also tells you upfront if anything is missing — so you don't make a wasted trip.
📋 How to Replace Your ID
Follow these four steps in order.
🚔 File a Police Report
Visit the nearest police station and report the loss, theft, or damage. Get the report in writing — you will need it later.
🏦 Pay the Replacement Fee
Pay UGX 50,000 at a bank and keep the receipt. Make sure your name appears clearly on it.
📝 Complete Form 11
Fill in Form 11 using black ink. Attach the police report and the bank receipt.
🏢 Visit a NIRA Office
Hand in your documents at the nearest NIRA office. Your photo and fingerprints will be captured for the new ID.

- 🏦 Bank payment receipt for UGX 50,000
- 📝 Completed Form 11
- 🆔 Any other form of ID, if available (passport, driving permit, voter's card)
🎫 What you'll receive: A new National ID card for persons aged 16 and above.
⚠️ Important: A police report is required even if your ID was damaged (not stolen). Also, the bank receipt must show your own name — receipts in someone else's name will not be accepted.
