📋 General Requirements

📝 Complete the correct form:
- Form 1 — for a change of information.
- Form 1A — for correction of an error.
🆔 Bring your National Identification Card (copy + original for verification).
🧑 Appear in person for photograph and biometric capture/update.
📄 Bring the required supporting documents for your specific change.
💰 Pay the prescribed fee where applicable: UGX 200,000.

✅ Free of charge: Where the error was made by NIRA, the correction is free and no supporting documents are required.

📋 Section A — Place of Origin, Parentage & Place of Birth

All items in this section are free of charge.

1. Correction of Place of Origin

📜 Statutory Declaration duly registered by URSB.
👥 Appear before the Local Citizenship Verification Committee of your village of origin and complete the Citizenship Verification Form.
✍️ Form signed by LC1 Chairperson, GISO/DISO, and an Elder/Senior Citizen; authenticated by the NIRA District Official.
📄 Copy of the National ID of either parent (or of a living blood relative if both are deceased).
👶 Birth Certificate, where available.
💰 Fees: Nil.

2. Correction of Parentage

📜 Statutory Declaration.
👶 Birth Certificate issued before the applicant registered for the National ID.
If unavailable, may be required: appearance of both the indicated and actual parent at NIRA CID office, or DNA test results (DGAL, Police Forensics, MBN, Makerere University Research Centre Lab, City Medicals), or a Court Order establishing parentage.
💰 Fees: Nil.

⚠️ Handled by the NIRA Legal Department.

3. Correction of Spelling of a Parent's Name

📜 Statutory Declaration.
🆔 Parent's National ID, or
👶 Birth Certificate.
💰 Fees: Nil.

4. Changing from Unknown to Known Parents

📜 Statutory Declaration registered at URSB.
📄 Birth Notification Record from your place of birth.
💰 Fees: Nil.

5. Changing from Known Parent to Unknown

🧬 DNA test results from an approved lab; or
⚖️ A Court Order.
💰 Fees: Nil.

6. Correction of Place of Birth

📜 Statutory Declaration.
👶 Birth Certificate, or
📄 Birth Notification Record.
💰 Fees: Nil.

7. Correction of Tribe

📜 Statutory Declaration confirming the correct tribe.
👥 Appear before the Local Citizenship Verification Committee in your village of origin.
✍️ Form signed by LC1 Chairperson, GISO/DISO, and an Elder/Senior Citizen; authenticated by the NIRA District Official.
📄 Copy of the National ID of either parent (or of a living blood relative if both are deceased).
💰 Fees: Nil.

8. Change of Name for a Child Below 18 Years

No Statutory Declaration, Gazette Notice, or Deed Poll is required.

May be applied for by:

👨‍👩‍👧 A biological or adoptive parent;
🛡️ A guardian; or
🤝 A person responsible for the supervision or welfare of the child.

Bring:

📝 Duly filled Form 7.
👶 Copy of the Birth Certificate.
⚖️ Adoption or Guardianship Order.
🆔 Copy of the National ID of the parent or guardian.
📄 Any document proving the relationship with the child.
💰 Fees: Nil.

📋 Section B — Name Changes (Fees Apply: UGX 200,000)

1. Omission or Adoption of a New Name

Other than by marriage or divorce.

📰 Copy of the Notice of Intention to Change Name published in the Uganda Gazette.
📜 Original Deed Poll duly registered with URSB.
📄 Any supporting document showing the name, where applicable.
💰 Fees: UGX 200,000.

2. Change of Name by Marriage or Divorce

📰 Copy of the Notice of Intention to Change Name published in the Uganda Gazette.
📜 Original Deed Poll duly registered with URSB.
💍 Marriage Certificate (by reason of marriage) or Decree Absolute (by reason of divorce), as applicable.
💰 Fees: UGX 200,000.

3. Adding Names Appearing on Earlier Official Documents

📜 Statutory Declaration duly registered with URSB.
One of the following:
- 👶 Birth Certificate issued before 1 January 2016;
- 🎓 Academic document issued before 2014; or
- 🛂 Passport issued before 2014.
💰 Fees: UGX 200,000.

4. Complete Change of All Names

Example: John Bosco Musoke → Michael Miles Semanda.

📰 Notice of Intention to Change Name published in the Uganda Gazette.
📜 Deed Poll duly registered with URSB.
🚔 Original Certificate of Good Conduct from Interpol.
🔎 CID Report from CID office NIRA HQ.
📄 Supporting documents: Academic document, Birth Certificate, Driving Permit, Baptism Card, Immunisation Card, or Voter's Card.
🎖️ For UPDF officers: clearance from the Joint Chief of Staff is required in place of the Police Clearance / CID Report and Interpol Certificate.
💰 Fees: UGX 200,000.

⚠️ Only handled at the NIRA Legal Department.

5. Removal of Nicknames, Pet Names or Titles

Examples: Dr., Prof., Mr., Mrs., Haji, Nalongo, Salongo, Rasta, Boss.

📜 Statutory Declaration duly registered with URSB.
📄 Supporting document: Academic document, Birth Certificate, Driving Permit, Voter's Card, or Immunisation Card.
💰 Fees: UGX 200,000.

6. Inclusion or Removal of Maiden Name

📜 Statutory Declaration duly registered with URSB.
💰 Fees: UGX 200,000.

7. Clarification of Initials on the Enrolment Form

📜 Statutory Declaration duly registered with URSB.
📄 Supporting document: Birth Certificate, Academic document, or Passport.
💰 Fees: UGX 200,000.

⚠️ Initials are not used as names in the National Identification Register.

8. Change in the Order of Names

📜 Statutory Declaration duly registered with URSB.
📄 Supporting document: Birth Certificate, Academic document, Passport, Immunisation Card, or Voter's Card.
💰 Fees: UGX 200,000.

9. Correction of Spelling Errors, Name Variations or Short Names

Examples: Mary / Maria, John / Yohana, Kenneth / Ken.

📜 Statutory Declaration duly registered with URSB.
📄 Supporting documents: Birth Certificate, Academic document, Passport, Immunisation Card, Voter's Card, Baptism Card, or Birth Notification Record.
💰 Fees: UGX 200,000.

10. Correction of Date of Birth

📜 Statutory Declaration duly registered with URSB.
📄 Supporting documents: Birth Certificate, Birth Notification Record, Baptism Card with church confirmation letter, UNEB Pass Slip/Certificate with verification letter from UNEB, Passport (valid or expired), Driving Permit, or Voter's Card.
💰 Fees: UGX 200,000.

Verification rules:

If the difference is 4 years or less → verified by CID in your district of origin/residence.
If the difference is more than 4 years → handled in Uganda at the NIRA Legal Department.

💡 The more credible documents you provide that consistently support the claimed date of birth, the stronger your application.

🎫 Note: Where the change involves name, date of birth, or place of residence, a new National ID reflecting the change is issued.
